Parents and Guardians,
If your student wants to play sports at NPMS next school year (2025-2026), they must get a sports physical. A sports physical MUST be dated on or after April 1, 2025, to be valid for the 2025-2026 school year. Students must renew this physical every school year.
To help prepare for the 2025-2026 school year, Hendricks Regional Health has set up physical days to provide high-quality, low-cost pre-participation physical exams.
To sign up for a sports physical, go to hendricks.org/sportsphysicals. Please direct any questions or issues to sportsphysicals@hendricks.org.
Sunday, April 13th at HRH Danville Hospital – 1000 E Main St, Danville {Enter through East Entrance}
Sunday, May 4th at Wabash College Knowling Field House {Enter off Jennison Street Parking Lot}
Once your student has received a sports physical, please create a FinalForms account and complete the forms there. Instructions on how to complete this can be found on our school website, https://www.nputnam.k12.in.us/o/npms. After you create the parent account, you will create the student account. Please use your student’s school email for the student account, not a personal email. Once you have created both accounts, you will then complete and sign all of the forms on FinalForms, and your student will also sign their portion of the forms. Then you will upload the completed (by your doctor) Physical Examination form.
